Radiacoustic and Ultrasound Imaging for Real-Time In Vivo Dosimetry and Closed-Loop Adaptive Radiotherapy.
Radiation therapy is highly effective, but current clinical systems often lack real-time feedback on where radiation energy is deposited in the patient, particularly in proton therapy, FLASH radiotherapy, and motion-affected treatments. This talk will introduce radiacoustic imaging, which detects radiation-induced ultrasound signals to visualize energy deposition in real time. I will present recent progress in radiacoustic imaging for in vivo dosimetry, proton range verification, and adaptive radiotherapy, including translational studies toward first-in-human proton therapy applications. I will also discuss an integrated radiacoustic and ultrasound imaging framework for closed-loop adaptive proton therapy, where ultrasound tracks anatomy and motion while radiacoustic imaging verifies dose delivery. Together, these technologies may transform radiotherapy into a real-time, feedback-guided precision treatment.
Dr. Shawn Xiang directs the TRUE Lab at the University of California, Irvine, where he is Professor of Radiological Sciences and Biomedical Engineering. His lab is pioneering in Radiacoustic Imaging (RAI) — a novel technology that listens to radiation itself to visualize energy deposition in real time. Dr. Xiang’s research centers on two major programs: RADAR (Radiacoustic Dosimetry for Adaptive Radiotherapy) and HEART (High-precision ElectroAcoustic-guided Rhythm Therapy). His team has secured over $15 million in NIH funding, translated radiacoustic imaging systems into the first-in-human proton therapy trials, and published more than 100 peer-reviewed papers, with more than 20 patents. He is the recipient of numerous honors, including the Best in Physics Award and Innovation in Medical Physics Award from the American Association of Physicists in Medicine, the NIH R37 MERIT Award, the American Cancer Society Research Scholar Award, and the Distinguished Investigator Award from the Academy for Radiology & Biomedical Imaging Research, and Emerging Innovation of the Year (2026) at UCI. Dr. Xiang is also the Co-Founder of SoundDose Inc., a company accelerating the clinical translation of radiacoustic imaging for precision radiotherapy.
